• The analysis side panel — nodes, tasks, timeline, and files — is now fully navigable by keyboard, with clear focus highlights and arrow-key movement between tabs, making it easier to work without a mouse.

• The report is now available right from the top navigation of the analysis, so the main output of your RCA is always easy to find and open.
• You can now save any analysis to your organization’s template library directly from the new tree editor.
• You can now mark a completed analysis as published from the side panel.
• The node details panel header is now color-coded to match the node type, so it’s easy to tell which node you’re editing at a glance.
• Easy Assistant can now suggest tasks for your analysis, and undoing a suggested addition re-enables the option to add it again.

• Copying a report to your clipboard now keeps its images intact when you paste it into Gmail or Google Docs.
• Timeline event times and revision history dates now display consistently in your local time across the timeline, editor, and report.
• “Fit to screen” now accounts for the open side panel so the tree is no longer partly hidden, and it animates smoothly

• The Easy Assistant now makes it clearer which node it is working on by matching the suggestion chip color to the node you have selected, with more consistent chip styling across node types.
• The Easy Assistant keyboard shortcut is now “E” (for Easy Assistant), and pressing it again closes the panel.

• Downloading a report as PDF or Word now works reliably; fixed an error that could stop the report from generating.
• Images now display correctly when viewing a report through a shared link.
• Fishbone analyses now place added items under the correct category instead of adding them as notes.
• Tree images now download at full resolution, matching the classic tree view.
• The Easy Assistant chat no longer gets stuck after a network interruption, so you can resend your message.
• Assigning a person to a task or to the analysis team by email no longer shows a confusing “user not added” warning.
• Cleared up inconsistent messaging about which file types are supported when uploading images to the Easy Assistant.
• The Easy Assistant no longer surfaces internal technical details when you ask which node it is focused on.
• The Easy Assistant chat no longer jumps to the wrong spot while you are typing a reply.
• Improved spacing between headings and bullet points in Easy Assistant responses.

• Easy Assistant gives sharper, more reliable guidance. It now pays attention to the node you have selected when you ask what to do next, recommends the right step for where you are in the analysis, encourages verifying the most likely failure mode before jumping to corrective actions, covers all node types more consistently, and no longer repeats duplicate suggestions.
• Non-image attachments such as documents and spreadsheets now appear as clickable links in the web report, so reviewers can download and open them directly.
• Removed the back button from the tree editor to make navigation clearer and more predictable.

         • Fixed formatting that could break in Easy Assistant replies while they were being written out, so responses now appear cleanly and load faster.

• Polished the Easy Assistant chat experience: the message box is now focused automatically when you open the panel, and message spacing and the send button are cleaner and easier to use.
• Added clearer guidance when pasting copied content into a legacy analysis, so it’s obvious what’s happening.
• Improved the Templates page to show a clearer “no results” message when a search doesn’t match any templates.

• Fixed AI “Suggest next node” recommendations not saving.
• Fixed standard users being unable to open shared organization templates on the new Templates page.
• Fixed PROACT templates with only a few nodes displaying too large.
• Fixed downloaded analysis images not matching the on-screen view, including misaligned lines and incorrect text sizes.
• Fixed Easy Assistant offering “Add” suggestions on completed or read-only analyses, where they couldn’t actually be applied.
• Fixed the support chat widget overlapping a button on the Templates page.

• You can now copy and paste nodes from one analysis into another, not just within a single analysis — making it much easier to reuse work across your RCAs.

• Fixed some analyses that couldn’t be edited, where newly added nodes or text disappeared immediately after being added.
• Fixed creation wizard issues: a blank screen when launching an analysis from the legacy editor, and event details not carrying over into the new event node.
• Fixed side panel issues: losing in-progress edits when switching between nodes, along with minor flicker and animation glitches when opening and closing panels and overlays.
• Fixed two Event Details timeline issues: the list opening scrolled to the bottom instead of the top, and the entry form drifting down the page as the list grew.
• Fixed template search not returning existing templates when searching by name.
• Fixed clicking a task from the Home or Tasks page not zooming to and highlighting its associated node in the tree.
• Fixed newly created analyses sometimes reopening in the older tree view instead of the new one.
• Restored the option to delete file attachments on analyses that are completed or published.
• Renamed the “Deleted” column on the Deleted Analyses page to “Deleted Date” so it matches the dates it shows.

• Expired user accounts now show a clear “your account has expired” message on the login screen instead of a confusing “Unable to establish connection” error.

• Fixed several issues in the new RCA wizard: launch failing when a Facility or Equipment was selected, multi-file attachment not working like the legacy version, and the Groups picker only showing a handful of groups on accounts with many.
• Fixed several attachment issues in the tree editor: the Files panel breaking its layout (and hiding the preview and delete actions) when a filename was long, and the over-50 MB upload error in Node Details showing the limit in raw bytes, duplicating itself, and lingering after clicking Choose File to retry.
• Fixed an app crash that occurred when a user account had no full name set.
• Fixed task icons below nodes not updating color when a task was marked complete or overdue.
• Fixed the RCA Team tab pairing each member’s name with the wrong person’s email.
• Fixed the Dashboard Facility filter displaying “N/A” after a selection instead of the chosen item’s name.
• Fixed the Settings dropdown getting cut off at higher browser zoom levels (125–150%) by moving its items into the new left-side settings menu.

• Indicators on nodes — including task and attachment icons — are now larger and easier to spot at a glance in the V2 analysis tree.
• When a node’s text exceeds the size limit, the error message now points users toward using a Note for longer content instead of cramming it into the node.
• The Change Type menu now disables the node’s current type so you can’t accidentally “change” to the type the node already is.
• Cleaned up the Event and Failure Mode toolbars by removing actions that don’t apply to those node types

• Fixed several issues in the new RCA wizard: validation errors when only Cost was entered, and a slightly cut-off file icon in AI Assisted mode.
• Fixed multiple V2 tree issues including disabled Notes and Tasks entry dialogs

• Fixed login-page issues including the Sign in with Okta and the Forgot Password page

• The team member search field in Event Details now has a clearer placeholder so it’s obvious you can add people there.
• The “Help” link has been removed from the settings dropdown — the Chat with Support and Email Support buttons are now the primary support paths.
• Node text length is now enforced in both the new and legacy editors, preventing errors when text exceeds the system limit.
• Groups dropdowns throughout the app are now searchable, making it faster to find the right group.
• The toggle visibility instruction now matches what’s actually clickable, removing a confusing mismatch.

• Fixed several issues in the new RCA wizard, including errors on launch, incorrect step navigation behavior, and layout problems on mobile.
• Fixed the RCA owner being temporarily removable from the team list, and corrected the Publish/status interaction so statuses can’t be changed after publishing.
• Fixed long group names overflowing in the RCA table and node details panel.
• Fixed multiple dashboard issues: the Facility filter failing to load data, search not working by analysis title, and the task flyout showing an empty state before data loaded.
• Fixed the www.easyrca.com redirect returning an error instead of loading the site.
• Fixed several cosmetic issues across the app including table borders, icon states, button overlap on mobile, and pagination display in the Deleted Analyses tab.
