Major UI Update: Groups, Settings Redesign & Workflow Enhancements
New Groups Management Table
Admins can now create and manage user groups directly from the app. The new Groups table lets you create groups, add or remove members, assign group admin status, and delete groups — all through a consistent table and flyout interface that matches the rest of the admin settings experience.
Redesigned Settings Pages
The Custom Fields, Assets, and Integrations settings pages have all been rebuilt with a modern table and flyout design. Custom Fields now displays a clean table with dedicated flyouts for adding, editing, and deleting fields. The Assets page features an updated upload form and reorganized FAQ layout. The Integrations page gives a clear overview of all configured webhooks (RCA, Task, New Tree, and New User) and Power BI integration, with easy copy-URL and test mode controls.
Task Links Now Navigate Directly to the Task
Clicking a task link anywhere in the app — from My Tasks, the Tasks page, or email reminders — now opens the analysis tree with the side panel focused directly on that task. No more searching for the right node.
New Features: Enhanced RCA Management and User Experience
Completed RCAs are Now Read-Only
Once an RCA is marked as completed, the analysis tree locks automatically to prevent accidental changes. Node text, types, and data become non-editable, and AI features are disabled. Users can still update the RCA status and completion date, reposition nodes, and toggle node visibility.
Select an Entire Branch at Once
You can now double-click any node to instantly select it and all of its descendant nodes. Use Ctrl/Cmd + double-click to add a whole subtree to an existing selection — making it easy to move, reorganize, or delete entire branches in one action.
Analysis Wizard Title Now Auto-Populates the Event Node
When creating a new RCA, the title entered in the Event Information step is automatically used as the event node’s text. No more typing the same thing twice.
Sign In with Okta
Okta users can now sign in directly from the login page using a dedicated “Sign in with Okta” button. It automatically routes you to your organization’s Okta login page based on your email domain.
New Analysis Tree Improvements
- There is now a toggle in the header dropdown menu to switch between Legacy and V2 Trees. Switching while viewing an RCA will redirect you to the same analysis in the other version.
- Return and Tab keyboard shortcuts now work like the legacy tree: while editing node text, Return creates a new child node and Tab creates a new sibling node.
- New button to download a screenshot of a V2 tree.
- Added buttons to adjust node text size.
- Color change for Contributing Factor nodes.
- All node types marked as untrue now display with a grayscale appearance.
- Connection edges now have a selected appearance.
- Selecting multiple nodes (cmd/ctrl + click) and pressing Delete will always prompt a confirmation dialog.
- The deletion confirmation modal now shows the number of child nodes and a count of notes, files, and tasks that would be removed.
- Hovering over the three dots on a hidden node shows a tooltip with information about that node.
- Side panel tasks, notes, and images are now ordered oldest (top) → newest (bottom).
- Fit to Screen now works when there are hidden nodes.

Smarter RCA Table: Custom Views, Status Indicators, and UI Enhancements
Conditional Formatting for Status
Statuses on the RCAs table now include color-coded formatting, making it easy to see the progress of each RCA at a glance and encourage timely completion.
New “Days Open” Column
A new column shows how many days an RCA has been open since it was created, helping users track progress and prevent analyses from becoming stale.
Column Reordering
You can now drag and drop columns to reorder them, giving you full control over how your RCA data is displayed.
Visible Filters Above the Table
The most commonly used filters—such as Group, Owner, Status, Facility, and Equipment—are now visible directly above the table for faster, more intuitive filtering.
